* Introduced visible_nodes for each level, got rid of root_level_visible.
* Levels can now have invisible nodes cached, updated all functions to
  take account of this fact. Some functions were also rewritten because
  of this and to correct the logic.
* All nodes on the virtual root path are referenced in order to keep at
  least the virtual root level cached in any child model.
* The rule that each referenced node should at least have one of their
  children referenced is now implemented correctly everywhere.
* Small bug fixes and (comment) updates.
