The echo test is precisely the problem: a steady sound from the microphone triggers echo cancellation on the same sound, albeit with a roundtrip latency, mixed in from the VoIP stream. Try using the speakerphone when the recipient is another device, which is ether in another location, or using headphones for audio output.
