Here is another specific but not so unusual use case:

When sleeping, especially with shift work, it may be necessary to wake and
respond to calls from some people - could be family, but I'm thinking
specifically of vulnerable family/friends where the calls could be a medical
emergency so you *must* wake on hearing the phone ring.  Anyone who's been in
that situation knows how you get trained to wake immediately when you hear the
phone ring.

At the same time, you don't want your sleep disturbed by anyone else calling or
even texts.  In that situation your sleep is probably even more precious.

Caller ringtones are great for that - or just as good, caller group ringtones.
You set everyone to silent, except the people you care about when sleeping.

But you wouldn't want a fixed ringtone configuration, because when you're awake
you obviously want to hear calls and texts from everyone else too - and you
might prefer a less "urgent" sounding tone from those vulnerable
family/friends.

On a vaguely related note, it'd be good to get a tone when receiving IMs from
some people but not others.

Not as closely related, and yet partially so: it'd be good to get a ringtone or
short tone (user configurable) when receiving the *first* IM in a chat sequence
which opens a window, but not one for every IM after that if you're actively
using the device.  But when idle in the middle of a chat, it'd be good to get a
ringtone again if there are pending IMs and you've not touched the phone for a
short idle time, implying that maybe you're not watching the screen and perhaps
far away from it, so you wouldn't notice a short tone either.  It's be superb
if you could flag specific open conversation windows as "I want to be notified
with a tone about messages pending that I apparently haven't noticed *in this
particular conversation*" - that's something I noticed recently when doing a
web chat with customer support that had no tones.
